Navigating the new ICB model

Integrated care boards (ICBs), first established by the Health and Care Act in July 2022, are undergoing significant transformation with the publication earlier this month of the Model ICB blueprint.

The model focuses ICBs on strategic commissioning, transferring other functions to providers, places, and NHS England.

What the changes mean for ICB boards

The new model requires ICBs to adapt quickly – operating cost plans are due today: 30 May 2025. Key areas of focus now include:

  • Strategic organisational alignment: Realigning ICB boards and ICB leadership structures to the new model, including necessary headcount reductions and meeting the £18.76 per head of population operating cost envelope. This includes assessing the viability of shared services for corporate functions (such as HR) across multiple ICBs and/or the viability of collaborations/ mergers.
  • Delegation: Reviewing and accelerating plans for transfer/ delegation to providers and places, which includes developing the right governance and oversight structures with the providers, places and within the ICB.
  • Risk management: Ensuring a smooth transition with robust governance.
  • Capability building: Developing expertise in the skills ICBs are expected to grow: population health, strategy, and commissioning.
