The Post Office, Grenfell and the Lampard Inquiries are all salutary and ‘before the deluge’ directors at all of these organisations never imagined that the way they were working would find itself scrutinised and multiple failings identified.
Yet the themes are blindingly repetitious: poor governance, non-executive accepting reassurance and not assurance, conflicts of interest, bad actors not addressed, the front line not listened to and groupthink, to name but a few.
